How do I enrol?
To enrol with HealthCentral, please download the enrolment form, complete and then email it back or drop it in to us.
The enrolment form will ask for your details such as name, age, date of birth, address and ethnicity. The information collected when you enrol is protected by the Privacy Act 1993 and the Health Information Privacy Code 1994.
